Avatar

A picture that falls back to initials, not to a broken-image icon.

The image can be missing or fail to load, and then initials are the answer.
Bits UI holds the loading and error state so we do not have to guess at it.

The fallback carries the name, so the avatar still has one without the
picture. Otherwise it is an empty circle to anyone who cannot see it.
